New Student Registration
Note: Students must be living with their parent or court-appointed legal guardian in the Woods Cross High School boundaries to register.
The following are required to enroll a new student:
1. A parent or legal guardian must be present at time of registration. Guardianship Status Form must be completed.
2. Bring photo ID.
3. Utah School Immunization Record as required by the Utah State Health Department. ENROLLMENT CANNOT TAKE PLACE UNTIL IMMUNIZATION DATES ARE PROVIDED. Dates required for the following:
DTP/DTAP/DT/Td
TDAP
Polio
Measles, mumps, rubella (MMR)
Hepatitis B
Hepatitis A
Meningococcal
Varicella (chicken pox)
4. Unofficial transcript and withdrawal grades from previous school.
5. Original birth certificate (a copy will be made).
6. Proof of residence/Procedimientos de Prueba de Residencia. Two forms of documentation are required.
8. Student Information Card/Formulario de Informacion del Estudiante.
9. Special Education (IEP) or 504 records. A copy of the IEP or 504 is required before classes can be scheduled in order to meet all accommodations.
10. McKinney-Vento Eligibility form/Elegibilidad McKinney-Vento.
Each State educational agency shall ensure that each child of a homeless individual and each homeless youth has equal access to the same free, appropriate public education, as provided to other children and youths. Davis School District has the responsibility to inform any homeless student of the services they are eligible to receive.

IMPORTANT: Student registration fees and/or fee waiver form/solicitud para la exencion del pago de cuotas must be taken care of in the main office at the time of enrollment.
